Fax or email your referral and we will call you to make an appointment.
Referral Letter
Please bring a referral letter from your general practitioner or other medical specialist.
A general practitioner's referral letter has a 12 months validity period. A specialist referral is valid for a period of 3 months.
Initial Consultation – Things to Bring

A current referral letter from you GP or other medical specialist

All hard copies of scan (X-rays, CTs, MRIs, PET scans or ultrasounds) and results

List of current medications

Medicare card, DVA card, Private Health Insurance information

Workcover or TAC approval letter, employer/insurer details and claim number.

Please arrive 10-15 minutes prior to your appointment time to register your details.

Cancellation Policy
Please give us at least 24 hours notice by calling during business hours so we can offer your appointment time to other patients on the waiting list.
Note: A cancellation fee may be charged at our discretion if you fail to turn up to your appointment without letting us know.
